Payment type: Prospective
In Model 4, the episode of care includes all services furnished during the inpatient hospital stay. CMS would make a single, prospectively determined bundled payment to the hospital that would encompass all of these services. Physicians and other practitioners would submit “no-pay” claims to Medicare and would be paid by the hospital out of the bundled payment. Participants will be permitted to share gains arising from better coordination of care.
How to Apply to Model 4

- The application Guidance document (PDF) for Models 2-4 is designed to offer applicants additional guidance in completing a strong application for the BPCI initiative. The Application Guidance document will clarify various sections of the BPCI online application portal.
- A Letter of Intent (PDF) was due by 5:00 pm ET, November 4, 2011. Letters of intent are non-binding, however an LOI is required to in order to apply to Model 4 of BPCI.
